SUMMER PROGRAM PARTICIPANT AGREEMENT

By signing this document, you acknowledge that you are giving up certain legal rights, including the right to sue.

1. PROGRAM OVERVIEW

The Sonovi Athlete Performance Lab is designed to improve athletic performance through structured training focused on:

• Strength Development
• Speed & Power Training
• Agility & Coordination
• Mobility & Flexibility
• Injury Prevention
• Movement Efficiency
• Athletic Performance Development

Participants will engage in supervised athletic training sessions led by Sonovi coaches.

2. ATTENDANCE POLICY

Athletes are encouraged to attend all scheduled sessions to maximize results and ensure consistent progress.

• Missed sessions cannot be made up, transferred, credited, or refunded.
• No refunds or credits will be issued for missed sessions due to vacations, scheduling conflicts, illness, or personal commitments.
• Consistent attendance is strongly recommended for optimal development.

3. REFUND POLICY

All program registrations are final.

Once payment has been processed:

• No refunds will be issued.
• No partial refunds will be provided for missed sessions.
• No credits will be applied toward future programs.

Exceptions may only be considered in cases of serious injury or medical circumstances that prevent participation and require physician documentation. Any exception is solely at Sonovi's discretion.

4. EMERGENCY MEDICAL AUTHORIZATION

In the event of an emergency, I authorize Sonovi staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that every reasonable effort will be made to contact me or my emergency contact prior to treatment whenever possible.

5. ASSUMPTION OF RISK

I understand that participation in athletic training and physical exercise involves inherent risks, including but not limited to:

• Muscle soreness
• Sprains and strains
• Falls
• Equipment-related injuries
• Other injuries associated with athletic participation

I voluntarily allow my child to participate and assume all risks associated with participation.

6. RELEASE OF LIABILITY

I hereby release and hold harmless Sonovi, its owners, coaches, employees, contractors, and affiliates from any claims, liabilities, damages, injuries, or losses arising from participation in the program, except in cases of gross negligence or willful misconduct.

7. PHOTO & VIDEO RELEASE

By participating in Sonovi training sessions, I authorize Sonovi to capture photographs and videos of the participant for marketing, promotional, educational, and social media purposes.

8. MEDIA RELEASE

I grant Sonovi permission to photograph, video record, or otherwise capture my likeness during training for educational, marketing, or promotional purposes, without compensation, unless I provide written notice revoking this permission.

9. PARENT DROP-OFF & PICK-UP POLICY

• Parents/guardians are responsible for ensuring participants arrive on time and are picked up promptly at the end of each session.
• Sonovi is not responsible for participants before or after scheduled program hours.
• Participants may not leave the facility early without prior parent/guardian authorization.
• To provide a focused training environment and minimize distractions, parents and guardians are not permitted inside the facility during program hours unless specifically authorized by Sonovi staff.
• Parents/guardians are welcome to drop off participants at the beginning of the session and return for pick-up at the scheduled end time.

10. CODE OF CONDUCT

Participants are expected to:

• Respect coaches, staff, and fellow athletes.
• Follow all coaching instructions.
• Use equipment safely and appropriately.
• Contribute to a positive training environment.
• Demonstrate good sportsmanship and professionalism.

Sonovi reserves the right to dismiss any participant whose behavior is disruptive, unsafe, or inappropriate. Dismissal from the program does not qualify for a refund.

11. PARTICIPANT INFORMATION

I acknowledge that:

• The information provided is accurate and complete.
• My child is physically capable of participating in this program.
• I have read and understand the policies outlined in this agreement.
• I voluntarily agree to all terms and conditions contained herein.
